The chronically homeless! Here is how to transition a chronically homeless person to housing.


The problem is that most chronically homeless individuals feel so hopeless that they cannot even fathom the possibility of housing. In our experience, they are set in their ways and in their comfort zone that was created many years earlier by whatever the circumstances were that forced them to homelessness. On top of that, some to most are plagued by alcohol, or the drug of choice, which is these days fentanyl-spread crack or heroin.

Transitioning chronically homeless individuals into housing requires a multi-step approach that addresses their immediate needs and sets them on a path to long-term stability. Here's a breakdown of the key steps:

  • Assessment and Outreach: Develop trust and friendship; if you are not trusted, you will not be able to get anywhere helping.

  • Conduct outreach programs to build much-needed trust and connect with homeless individuals. Assess their needs, including mental and physical health, income, and any disabilities.

  • Housing Placement: Match individuals with appropriate housing options, such as permanent supportive housing or transitional housing.

  • Prioritize those with disabilities or chronic health conditions.

  • Support Services: Provide case management to assist with tenancy, budgeting, and daily living skills.

  • Offer mental health and addiction treatment services as needed.

  • Employment and Benefits: Help individuals secure employment or job training programs. Assist with applications for Social Security, disability benefits, and healthcare.

  • Long-Term Stability: Monitor progress and provide ongoing support to prevent future homelessness. Foster a sense of community and belonging for individuals.

This comprehensive approach recognizes that housing is essential but not sufficient. By addressing the underlying causes of homelessness and providing ongoing support, communities can help chronically homeless individuals rebuild their lives.

Homeless Long Island is homeless, however we work out of our car, our house and our church. We were using our church as an address, unfortunately too many people were showing up thinking the church was a shelter.
